Below are two forms which may be downloaded in order to expedite the processing of your insurance claim and/or the repairs to your vehicle.
The “Completed Work Claim Form” allows us to proceed with with the repairs and authorizes the responsible insurance company to make payment to us.
The “Insurance Vehicle Release Form” allows us to release your vehicle to the insurance company or other third party.
